We by default access your Facebook basic account information, including your name, email, gender, birthday, current city, and profile picture URL, as well as other information that you choose to make public. Physician Dispensed Medications Available for GoMED Patients GoMED now offers patients the option of having medications that appear on the “GoMED Formulary” dispensed and provided during your encounter. At this time GoMED, LLC does not accept assignment and/or bill your insurance for dispensed medications. Physician dispensed medications are 100% patient responsibility. We offer this service as a patient convenience. GoMED’s formulary is limited and not all medications are available. Controlled substances do not appear on the GoMED formulary. GoMED, LLC will collect payment for any dispensed medication at the time the medication is dispensed. GoMED accepts cash, checks, and all major credit cards. GoMed, LLC will continue to provide electronic, or written prescriptions that can be filed by your local pharmacy, and may be covered under your pharmacy benefits, if available. Alternative sources of the medication for which you have been ordered. Any pharmacy covered under your health benefit plan. My signature affirms that GoMED, LLC has provided me with options available to receive medications necessary for treatment of a medical condition diagnosis by a GoMED, LLC healthcare professional.
